This position may work remotely up to two (2) days per week.
As the Population Health Clinical Pharmacist, you will provide Population Health Management Services to patients of the Medical Group of ChristianaCare. The Clinical Pharmacist is responsible for completing comprehensive medication reviews for eligible patients, as well as ensuring that performance targets are met or exceeded for medication-related measures across value-based contracts. Collaborates with providers and patients to ensure patients are meeting applicable measures.
Principal Duties and Responsibilities:
- Designs and implements clinical programs
- Collaborates with patients, providers, and payer-based pharmacists to optimize outcomes in risk-based contracts with a focus on CMS STARS and HEDIS quality measures
- Provides comprehensive medication management services and annual wellness visits to eligible patients
- Utilizes risk stratification tools to identify targeted high-risk patient population
- Reviews for contraindications/precautions, the patient’s current status of health, and drug profiles and patient’s medical records for drug-drug, drug-food, drug-laboratory, and drug-disease state(s) potentials.
- Documents drug use and compliance patterns, adverse drug effects, drug use and compliance patterns
- Coordinates activities with health care team to perform enhanced medication reconciliation on high-risk populations and patients or as indicated by assessment.
- Participates in research design, data analysis, and publication of study results as a preceptor for resident research projects.
- Contributes to the pharmacy and medical literature (e.g., case reports, letters to the editor, and therapeutic, pharmacokinetic, and pharmacoeconomic reports, peer reviewer).
- Participates in regional, national, and international meetings for the pharmacy and medical community.
- Provides an interprofessional experience in training and mentoring pharmacy students and residents through experiential ambulatory care rotations as assigned
- Travels to practices to meet department needs
Education and Experience Requirements:
- PharmD from an ACPE accredited School of Pharmacy Required
- ASHP-accredited PGY-1 Pharmacy Practice Residency required or equivalent experience (3 years of clinical pharmacy experience)
- ASHP-accredited PGY-2 Ambulatory Care Residency preferred
- Experience in ambulatory care and/or population health preferred
- BPS board certification preferred
- Current licensure (or eligible to reciprocate) in the State of Delaware
- May require licensure in additional states in order to provide virtual care to patients

Hourly Pay Range: $58.26 - $93.22
This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
