Position Details:

Clinical Pharmacist – Hospital (Overnights, 7-on/7-off)

$10,000 Sign-On Bonus (Paid at 6, 12, 18, and 24 months)

Position Summary

The Clinical Pharmacist collaborates with the healthcare team to ensure the safe, effective, and evidence-based use of medications. This role serves as a key drug information resource for physicians, nurses, and patients while supporting high-quality patient care. The pharmacist performs all responsibilities in alignment with organizational mission, values, and service standards.

Key Responsibilities

Clinical Practice

  • Review patient medical records to assess the appropriateness of medication therapy.

  • Evaluate patient conditions to ensure comprehensive treatment of all clinical issues.

  • Develop and implement medication management plans that optimize outcomes and minimize adverse effects.

  • Monitor therapy through chart review, patient profiles, and collaboration with care teams; intervene as clinically appropriate.

  • Assess treatment outcomes and modify therapy based on patient response.

  • Provide recommendations on dosing, administration, drug interactions, contraindications, and formulary alternatives.

  • Deliver Medication Therapy Management (MTM) through patient assessment and collaboration with prescribers.

Collaboration & Communication

  • Partner with physicians, nurses, and interdisciplinary teams to ensure optimal patient care.

  • Serve as a drug information resource for clinical staff and patients.

  • Maintain strong, professional relationships with healthcare providers.

  • Communicate effectively across shifts to ensure continuity of care.

Operational & Professional Duties

  • Evaluate medication orders for clinical appropriateness and operational efficiency.

  • Prioritize workload effectively in a fast-paced overnight environment.

  • Support fiscal stewardship through responsible resource utilization.

  • Participate in investigational protocols, quality assurance initiatives, and process improvement efforts.

  • Document clinical interventions, medication plans, and patient outcomes accurately.

Education & Development

  • Precept pharmacy students and residents as assigned.

  • Provide in-services, presentations, and education to staff.

  • Participate in continuing education and maintain licensure requirements.

  • Support competency development within the department.

Compliance & Quality

  • Ensure adherence to all regulatory, accreditation, and organizational standards.

  • Participate in medication utilization evaluations and adverse drug reaction reporting.

  • Complete required inspections, safety checks, and documentation (e.g., expiration dates, floor stock).

  • Maintain compliance with safety, confidentiality, and risk management policies.

Technology & Systems

  • Utilize pharmacy and clinical systems (e.g., Epic) for order verification and documentation.

  • Access drug information databases (e.g., Micromedex) to support clinical decision-making.

  • Operate pharmacy automation systems (e.g., Omnicell, ScriptPro, McKesson) as required.

Additional Expectations

  • Demonstrate professionalism, strong communication skills, and excellent customer service.

  • Maintain appropriate appearance, hygiene, and adherence to dress code and PPE requirements.

  • Work efficiently under time-sensitive conditions to meet deadlines and patient care needs.

  • Participate in departmental initiatives, committees, and policy development as requested.
